I made up two different surveys for your convenience

One long and one short

These two surveys use Javascript for newer Browsers of Netscape and IE

The reason for this is I lost my CGI-BIN script and access

The long survey has lots of extra information for our Database

The short survey has just the basics to list your dojo on K-1

For your convenience pick the survey you wish to do
